To help save paper, our packets are designed to be printed two-sided. If you are unable to view or print the paperwork, you can also stop by our office to pick up a printed packet.


NEW STUDENT ENROLLMENT (Benchmark)

If your child will be attending Benchmark as a new student in the 2019-2020 school year, including the Summer Program, as either a typically developing OR a special needs student, please complete the 2019-2020 Enrollment Packet. Please carefully read and complete all pages and submit your completed packet, along with all required documentation, by postal mail or in person.

NOTE: This is NOT the enrollment paperwork for the Universal Pre-K program in the Watertown or Carthage Central School Districts! Please scroll down to see those.

ENROLLMENT CHANGES (Benchmark)

A new enrollment agreement must be completed every time there is a change in your child’s services or schedule. For example: changing from the 3-hour program to the 5-hour program or changing from 2 days a week to 5 days a week.


UNIVERSAL PRE-K ENROLLMENT (Watertown & Carthage)

Benchmark Family Services administers Universal Pre-K programs in the Watertown and Carthage Central School districts. Enrollment is based on your place of residence and runs through the school district.

  • Watertown City School District
    Watertown offers Universal Pre-K programs on a full-time (5 hour) basis for 3-year-old and 4-year-old students at Ohio Street Elementary, Knickerbocker Elementary, and the Learning Center (formerly the Armory), or on a part-time (3 hour) basis for 4-year-old students at Benchmark Family Services.

    To enroll, you must first enroll with the Watertown City School District. AFTER your child has been accepted into the program, please complete the 2019-2020 Watertown enrollment packet and return it to Benchmark (NOT the School District) along with all required documentation.

    The Watertown City School District does NOT provide bus transportation to students, but free or reduced rate school lunches are available to qualifying students in some locations.

  • Carthage Central School District
    The Carthage Central School District offers Universal Pre-K programs on a full-time (5-hour) basis to 4-year-old students at the Black River, Carthage, and West Carthage Elementary Schools.

    Carthage enrollments are based on a lottery system, NOT a first-come, first-served system. Our third Open Enrollment period for Carthage runs 8 July to 29 July and all applications must be received within this period to be considered.To enroll, you must download the enrollment packet from the School District website, and return the completed packet to Benchmark (NOT the School District) along with all required documentation.

    The Carthage Central School District provides bus transportation for all students who are 4 years old on their start date. (NOTE: Students who are still 3 years old at the beginning of the school year will have to be transported by their parents.) Free or reduced-rate lunches are available to qualifying students.


EMERGENCY CONTACT INFORMATION

It is important for schools to have up-to-date contact information for a child’s family or guardian and emergency contacts. If you move, change your phone number, or need to add or remove someone from your emergency contact list, please complete a new emergency contact form and bring it to the school as soon as possible.
